Pick the visit that fits you, a first visit or a follow-up, in person or virtual. Then choose either Las Vegas location and any available provider.
Complete your intake forms before you arrive, and bring a valid ID and your insurance card. Copays and deductibles are due at the time of your visit.
Your first visit covers your full health story, including labs, medications, lifestyle and goals. You leave with a personalized care plan and clear next steps.

See your provider in person at our Central Las Vegas or Centennial Hills clinic, or book a virtual visit from home. Our North Las Vegas clinic opens in January 2027.
HEALOR accepts many plans, including Medicare, Aetna, Anthem BCBS, Cigna, Humana, Tricare, United Healthcare and all PPO plans. Health Plan of Nevada, Beech Street and Culinary are not accepted.
